To Life!™ Lifestyle Change Program
Payroll Deduction Plan
Methodist Health System
Dear Methodist Employees,
Welcome to the new partnership of the Methodist Health System and the To Life!™ Lifestyle Change and Weight Loss Program! To Life!™ is a nationally recognized one year (minimum) lifestyle change program for weight loss and weight loss maintenance that produces results! Safe, effective weight loss doesn’t have to be hard. You just need a plan and a place to practice until you become confident again in your ability to take good care of yourself. Our educational system of exercise, nutrition, behavior change and support, if embraced and practiced, will help you feel better and be much, much healthier. Please feel free to explore this website for a closer look at To Life!™.
In addition to the payroll deduction, all Methodist employees and family members will receive a 10% tuition discount. You can participate in the program and spread your payments out over one full year by signing up for the employee deduction plan. This means that instead of the usual lump sum payment at the beginning of the Program, you may elect, through payroll deduction, to have those payments deducted from your paycheck in 24 equal installments. The forms for payroll deduction can be filled out in our office at the time of your first appointment with Dr. Pincus.
There are several easy ways to get started:
- You can attend one of the monthly “Meet the Doctors” information sessions sponsored by the Methodist Weight Management Institute. Both the To Life!™ medical weight loss program and obesity surgery will be discussed. Look for flyers announcing the event or check the “RSVP for Information Session’ of this web site for dates and times.
- A brochure describing the To Life!™ program is now available from Human Resources at all Methodist Health System hospitals.
To Life!™ is a 12 month program. This is because meaningful weight loss and weight loss maintenance take time. Except as otherwise agreed in writing, the fees are non-refundable. If you choose to payroll deduct all payments will continue until the balance is paid in full.
